Disclosure climbs a ladder scaled to sensitivity: STANDING → CHALLENGE → HUMAN → MULTIFACTOR. The ladder now stands end to end — A1 the Warden proves witnessed data, A2 the Sovereign co-signs on a direct Warden↔Sovereign channel, and a registry-governed factor-2 step-up runs out-of-band on a direct Warden→Signet channel where the Signet prompts 'Approve action?' before anything leaves. Tested live (e2e:evidence · -stepup · -direct). Realises the top rung of C96.
